If you cannot keep your appointment, please notify us in advance so we can reschedule you to a more favorable time. We look forward to seeing you. New patients are asked to arrive 15 minutes prior to their appointment time so that you can complete the paper work . If you go to the patient portal and complete our new patient forms, ahead of time, it will help make the check-in process much more efficient.

Insurance: We file most insurance and help you file your claim. We also handle any insurance questions that may arise. We do not accept Medicaid or Medicare. Our staff is extremely knowledgeable about health insurance and filing claims. Please bring your insurance card with you to your next scheduled appointment. A copy of your insurance card is required in order for us to properly submit your medical claims for payment. If you do not have insurance, you will be considered “self-pay” and will be responsible for the payment at the time of service
